Life Diagnostic Centre ("we", "us", "our", "the Centre") respects your privacy and is committed to protecting the personal and health-related information you share with us — whether you visit us in person, call us, submit a message through this website, or use our online Report Portal to view or download a laboratory report. This Privacy Policy explains what information this website collects, how it is used, how it is protected, and the choices available to you. It applies only to this website and does not cover information held in our internal laboratory information system, which is governed separately by our clinical record-keeping and confidentiality practices.

1. Information We Collect

a) Information you provide to us

• Report Portal access: your Lab No. / MR No. and the confidential PIN printed on your receipt, used only to locate and unlock your own report(s).
• Contact form: your name, phone number, email address, and the content of your message.
• In person or by phone: any information you give our staff when booking or discussing a test — this is recorded in our laboratory system, not on this website.

b) Information collected automatically

• Technical details such as IP address, browser type, device type, and general access times, used for security and to keep the site running reliably.
• A record of report searches and successful/failed PIN attempts (access and verification logs), used to detect misuse and protect your report from unauthorized access.
• Essential cookies/session data needed for the site and portal forms to function (see Section 4).

c) What we deliberately do not collect here

This website is not a full electronic health record. We do not store your complete medical history, clinical notes, or structured test results on the web server — only the PDF report(s) your Lab No./MR No. and PIN are authorized to unlock, and only for a limited time (see Section 6).

2. How We Use Your Information

• To retrieve and display the correct laboratory report to the correct patient.
• To respond to enquiries submitted through the Contact page.
• To detect, investigate, and prevent unauthorized or fraudulent attempts to access reports.
• To maintain, secure, and improve this website.
• To meet legal, regulatory, or record-keeping obligations that apply to a diagnostic laboratory.

We do not use your information for advertising, and we do not sell, rent, or trade your personal or health information to any third party.

3. How Your Report Is Kept Secure

Access to your report requires both your Lab No./MR No. and the confidential PIN issued on your receipt or via your direct report link. Repeated failed attempts trigger a security check to slow down automated guessing, and every access attempt — successful or not — is logged. Reports are only made available online after they have been verified by our laboratory staff, and each report automatically stops being accessible online after a set number of days (see Section 6). Please treat your PIN like a password and avoid sharing report links with anyone other than your treating doctor or someone you trust to see your results.

4. Cookies

This site uses only the cookies/session data strictly necessary for it to work — for example, keeping you securely logged into the Report Portal for the duration of your visit, and remembering that you passed a security check. We do not use third-party advertising or cross-site tracking cookies. If a third-party map or widget is ever embedded on this site, that provider's own cookie practices may apply while that content is loading, independently of this Policy.

5. Sharing and Disclosure

We do not sell your information. We may share limited information only:

• With the doctor who referred you, so they can review your report through our system.
• With trusted service providers who help us run this website (e.g. our hosting provider), who are only permitted to use the information to provide that service.
• When required by law, court order, or a lawful request from a competent government or regulatory authority.
• To protect the rights, safety, or property of the Centre, our patients, or the public — for example, investigating suspected fraudulent report access.

6. Data Retention

Online reports remain downloadable for a limited number of days after verification, as configured in our system; after that, the report is no longer accessible through this website, though the original record remains safely on file at the Centre. Contact-form messages and access/verification logs are kept only as long as needed for support, security, and audit purposes, and are periodically reviewed for deletion.

7. Data Security

This site is served over an encrypted (HTTPS) connection. Report PINs and staff credentials are never stored as plain, readable text. Access to the underlying systems is restricted to authorized staff, and every verification and access event is logged for audit. No method of transmission or storage is 100% secure, but we take reasonable, industry-standard steps to protect your information against loss, misuse, and unauthorized access.

8. Your Rights and Choices

You may, at any time:

• Ask us what information we hold about you and request a copy of it.
• Ask us to correct inaccurate information.
• Ask us to delete a contact-form submission you sent us.
• Ask a question about how your report was accessed online.

Send these requests using the details in Section 13. We may need to verify your identity before acting on a request, particularly one involving a laboratory report.

9. Children's Privacy

This website is not directed at children. Where a report belongs to a minor, it is expected to be accessed on their behalf by a parent, guardian, or treating doctor using the PIN issued for that report.

10. Third-Party Links

This site may link to third-party services (for example, a map or a social media page). We are not responsible for the privacy practices of those third-party sites, and we encourage you to review their policies separately.

11. Changes to This Policy

We may update this Privacy Policy from time to time to reflect changes in our practices or for legal reasons. The "Last updated" date at the top of this page shows when it was last revised. Continued use of this website after a change means you accept the updated Policy.

12. Governing Law

This Policy is governed by the laws of Pakistan. Pakistan does not yet have a single, comprehensive data-protection statute in force; unauthorized access to, or misuse of, electronic data is addressed under the Prevention of Electronic Crimes Act (PECA), 2016 (as amended). As a licensed diagnostic laboratory, we also follow the patient-confidentiality expectations of the Punjab Healthcare Commission and other applicable healthcare regulations. Where our practices are not yet mandated by local law, we aim to follow internationally recognized privacy principles as good practice.
